    Pet Release Form

    AUSSIE PET MOBILE® requires each pet being groomed to have current rabies vaccination.  We also strongly advise that each pet be vaccinated against distemper, hepatitis, parvovirus, corona, leptospirosis, and bordetella prior to being groomed.

    AUSSIE PET MOBILE® has the right to refuse grooming any animal that may be a threat to itself or any person.  If we are not able to take a cat out of its carrier or get a muzzle on a dog that may bite, then we may have to refuse it for its own protection and ours.  In these cases, a minimum fee of $50 will apply to cover our costs.  The actual amount will depend on the length of time the groomer was at the home.

    AUSSIE PET MOBILE® does not use any type of tranquilizers on any pet.  If your veterinarian recommends tranquilizers prior to grooming, it will be the client’s responsibility to administer the medication and AUSSIE PET MOBILE® bears no responsibility for any resulting side effects.

    Shaving a pet often exposes pre-existing skin conditions.  If an animal is badly matted, you may find sores and tender skin under those mats.  Many times the skin is so sore that the clippers may further irritate the skin when removing the mat. We will go slowly and be extremely careful on a severely matted area but it is important to realize that the sore or irritated skin was caused by the mat and not the mat removal process.

    We also use natural and chemical solutions and procedures to treat your pet for fleas and ticks.  Some animals are sensitive to these solutions and in some rare cases, may experience an allergic or other serious reaction.  We monitor your pet closely when administering these solutions and will notify you immediately if any such reaction occurs.

    Our groomers will also take pictures of the pets that they groom to share with the company owners. These pictures are used to monitor grooming quality, retain for future grooming reference and to share best practices across our team.  We also post pictures on our website and Facebook/Twitter accounts to allow new clients to see the quality of our service and to highlight the adorable pets that are entrusted to our care.

    Trip charge

    It is the responsibility of the owner to have the pet available for grooming at the time of the appointment.  Cats should be contained in their carrier prior to the groomer’s arrival, so they are readily available at the time of the appointment.  If the groomer has arrived and the pet cannot be secured within 10 minutes of our arrival, the appointment may need to be rescheduled and a $50 trip charge will apply.

    A minimum trip charge of $50 also applies if no one is home or access to the home has not been provided for a scheduled appointment.

    We accept cash, check or credit card (Master Card, Visa, Discover).  Returned checks are subject to a $20 service charge.

    RELEASE

    I am the owner/caregiver of this/these pet(s) and have read and understood the information printed above.  I realize that pet grooming has the rare potential to cause injury or allergic reaction to my pet(s), but I consent to and authorize the grooming of my pet(s).  I also release AUSSIE PET MOBILE® and its employees, franchisees or other representatives, from any responsibility or liability arising out of the performance of those services.
